Permission For An Unrelated Applicable Adult To Travel To Competition Alone With Minor Athlete

I, ________________________________________, legal guardian of ________________________________________, a minor athlete give express written permission, and grant an exception to the Minor Athlete Abuse Prevention Policy for ________________________________________ (minor athlete), to travel with  ________________________________________(applicable adult)  from ________________________________________ (point of origin) to ________________________________________ (point of destination) from __________ to __________ (dates of travel to competition).

I acknowledge that ________________________________________ (minor athlete) cannot share a hotel room, sleeping arrangement or any overnight lodging location with ________________________________________(applicable adult) at any time.
